Tools to Disseminate Techology to Manage Chronic Illness

The Center for Technology and Aging has developed four toolkits , called ADOPT toolkits, to help organizations incorporate technology into their processes for providing care to older adults and others with chronic illnesses.  The four toolkits include 1) technology for remote patient monitoring, 2) medication optimization, 3) managing care transitions, and4)  managing chronic illness with the assistance of mobile devices.  The toolkits actually assist a healthcare organization integrate the technology into their workstream by walking them through the steps necessary to manage the change successfully.  Sue Sweeney, Chair, Gerontology Department, Madonna University
